  • This is an official grievance form which is submitted to The Dean of Students (Student submission), The Dean of Faculty (Faculty submission), Human Resources (Staff submission or general grievance) for complaints towards any Darul Qasim official, faculty, staff or fellow student(s). If the complaint is towards The Dean, please email The President or Human Resources at hr@darulqasim.org without the need to complete this form. Prior to submitting a grievance, please completely read through the "Grievance Policy" section in the Student Life or Employee handbook.

    Important Notice
    This is an official grievance form used for complaints towards any Darul Qasim official, faculty, staff or fellow student(s). It is extremely important the information is accurate, to the point and factual. Do not use more than 300 words to articulate the facts or the form will be rejected and deleted. Documented proof may be provided upon any official follow-up discussions after the form submission. Once you have submitted this form, please do not pursue, communicate or contact anyone verbally, over text, email or any other means regarding the grievance until you have been contacted by a Darul Qasim Official.
